Be it a pregnancy scare or unsatisfying sex, some men just struggle to last long enough in bed. Premature ejaculation is a common problem for Singapore men. Fortunately, there are medical ways to increase your “sex time”. PE is a condition that is not commonly seen by your everyday GP. But you would be surprised that the treatment is already on their shelves. So brace yourself, as we uncover the secret world of sexual stamina that even your doctors may not know.
But if you not yet ready for prescription-based treatment, you can explore some natural methods that can help increase your sex time or other treatments available in Singapore-based clinics.
Do I have premature ejaculation?
PE refers to early ejaculation that happens upon penetration, and it is a common problem especially for Asian men.
A health survey found that 1 in 3 Asian men suffer from PE. 80% of respondents reported that PE was affecting their relationships negatively. And this applied to both men and women alike.
Most men who suffer from PE have all 3 of the following:
(1) ejaculation in less than 2 minutes of penetration;
(2) inability to delay ejaculation during intercourse; and
(3) psychological distress and/or avoidance of sexual intimacy.
The International Society for Sexual Medicine recognizes PE as a medical condition. Hence, there are solutions to help you last longer in bed.
Local anaesthetics – Numb it to prolong your sexual stamina
Ever tried Durex Prolong condoms? It works, because it contains an anaesthetic agent. And it also leaves you with a tingling sensation.
In a 2013 review study, local anesthetics helped male participants improve sexual satisfaction with a significant increase of average time to ejaculation by ~ 5 mins.
For gels, simply apply the gel to the tip of your penis for 10 to 15 minutes prior to sex. Do not “overdo” as a prolonged application (30 to 45 minutes) has been reported to cause loss of erection due to numbness.
For sprays, you can spray the medication to your penis for 10 to 15 minutes before sex.
Some men prefer spray to gel as it might be easier to apply and clean up.
While having sex, the residual lidocaine gel might result in numbness of your partner’s vagina. Therefore, it is important to wash off the gel right before sex to have a pleasurable sexual experience.
However, if you don’t like the feeling of having “something” on your genitals, fret not! Tablets are also available.
Prescription to prolong by modifying your brain chemicals
Serotonin is a key hormone in our brain that has control of the ejaculation. Men with higher levels of serotonin tend to climax later.
SSRI works by increasing serotonin levels. This mechanism helps to delay ejaculation.
Many SSRIs have been shown to be effective PE treatment in clinical studies. Having the same essential mechanism, they differ in terms of dosing frequency (on-demand vs. regular), onset of action, time to elimination, etc.
To date, only one SSRI, Priligy (dapoxetine), is FDA licensed for PE treatment. Other SSRIs, although proven effective in clinical trials, are used as off-label.
Priligy – the only FDA-approved for premature ejaculation
Priligy has been approved for the treatment of PE in over 50 countries worldwide, including Singapore.
In an evidence-based review of its effectiveness, Priligy was shown to increase time taken to ejaculation from 0.9 minutes to 3.1 minutes (30 mg dose) and 3.6 minutes (60 mg dose), compared to 1.9 minutes (placebo). Besides, men taking Priligy reported a significant improvement in perceived control over ejaculation with lower emotional distress.
As Priligy rapidly “enters” and “leaves” our body in comparison to other SSRIs, it is taken on-demand (instead of daily) approximately 1 to 3 hours prior to sexual activity. You will start with the recommended (lower) dose, with a higher dose given only if the response is insufficient.
Priligy is considered to be safe for casual use during sex, so you do not have to worry about taking it as a long term solution for premature ejaculation.
Off-label treatments of premature ejaculation
Off-label use of medication refers to the use of a medication for another condition other that what its label says its for.
Other SSRIs have been demonstrated to provide significant benefit in delaying ejaculation too. Most popular off-label SSRIs are paroxetine and sertraline
These off-label SSRIs may be used on-demand or taken regularly.
Sounds complicated? Don’t worry. Share your preference and consideration with your doctor – he or she will advise you on the best choice.
While SSRIs are effective for PE, they are not without undesirable effects – the most common ones include nausea, headache, dizziness, and fatigue. Be sure to discuss any particular concerns with your doctor.
Viagra for premature ejaculation? Treat concurrent ED too
A good proportion of men with PE also struggle with erectile dysfunction.
When it takes more effort to sustain an erection, it naturally overstimulates your organs, hence bringing you to climax quicker.
So, when getting to the root of PE, ED is the “culprit” to be treated first.
The good news is that erectile dysfunction is highly treatable with many forms of effective treatments – the most widely used medications are oral PDE5 inhibitors, like Viagra and Spedra.
These medications work by increasing blood flow to the penis to sustain the erection.
If your ejaculatory problems still persist despite having a good erection, your doctor may advise you to take Priligy in combination with a PDE5 inhibitor.
